The false premise it to assume something is there which is not there. It is to confuse what is unknown as being known. It is to assume Lumen Gentium 16 is an exception to the strict interpretation of EENS i.e LG 16 is considered physically visible  when it really always refers to an invisible person.But it has to be physically visible for it to be made an exception to EENS.

It is seen as a real person, a non  Catholic saved outside the Church. This is false. Since LG 16 always refers to a hypothetical and theoretical case only. 
So LG 16 is interpreted with a false premise and then a false inference is made about Vatican Council II.
Vatican Council II is considered a break with Tradition since LG 16 refers to a known non Catholic saved outside the Church, i. e someone known in the present times who is saved without the Sacraments. EENS is made obsolete.
Without the false premise LG 16, LG 8, UR 3, LG 14, NA 2,GS 22 etc would only be theoretical and so would not contradict EENS and exclusive salvation in the Catholic Church.-Lionel Andrades
